      Community Health Integration Expo (Vendor Sign-up)

      Greetings, Central MN Mental Health Center (CMMHC) and CentraCare Health are hosting our annual Health Integration Expo. The Expo will be held at the St. Cloud River’s Edge Convention Center on July 24, 2018 from 9:00 AM to 2:00 PM. We cordially invite you to participate as a vendor at this event. You may register as a vendor on Event Brite for a fee of 50.00. This fee includes your table, lunch and parking. (Electrical and/or wireless access are included if needed). CMMHC, a nonprofit organization, has been providing Mental Health Services to adults, children and families in the communities we serve for almost 60 years. We provide a wide array of Mental Health Services based on the needs within the four counties we serve, Benton, Sherburne, Stearns, and Wright. CentraCare Health is a not-for-profit health care system that provides comprehensive, high-quality care to people throughout Central Minnesota. The collaborative network includes hospitals in St. Cloud, Long Prairie, Melrose, Monticello, Paynesville and Sauk Centre, more than 25 clinics and nursing homes and senior housing in several communities. Through our Health Integration Program, CMMHC is currently partnering with CentraCare Clinic Midtown to provide Mental Health and Primary Care services at our Midtown site. Our target population is adults with serious mental illness and/or co-occurring substance use disorder, who have, or are at risk for, chronic diseases. People with serious mental illness are dying an average of 25 years earlier than the general population due to a lack of medical care, follow up services and social determinants. This is equal to the life expectancy of populations in third world countries. Knowing what services are available and how to access them is often a barrier. Our goal for the Health Integration Expo is to provide a wide array of informational and interactive opportunities to this high-risk population and their families. By inviting our community to participate in this event, we are hoping to reduce the stigma of mental illness and increase understanding of the importance of treating both body and mind together. Our clients’ experiences are what matters and they can best explain how our Health Integration Program has improved their quality of life: “I appreciate being able to walk across the hall and schedule an appointment for my bad knees. I get my mental health services here and I can get in to see Nicole [our medical provider] within a few days.” “The first time I met you [peer wellness coach] I was bawling my eyes out. I realized on my way home that this moment was a turning point in my life. I can do this. I am worthy. I want recovery. I need your support.
