The #1 OSHA citation in 2008 was: Scaffolding - Construction (1926.451), with 7,027 violations! 

Citations in this standard include: failure to have a competent person on site while scaffolding in use: failure to provide adequate training and failure to provide adequate fall protection. 

Are your employees trained?  According to OSHA, each employer shall have all employees who perform work while on a scaffold trained by a qualified person and be able to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards.
